If the Virtual Learning Request Form is not submitted by the deadline, your child will be expected to attend school face-to-face when their assigned grade level transitions back to the building. This completed form is required for your child to remain a virtual learner.

The form will be accepted with an electronic signature if it is verified by your child’s teacher. The form can also be submitted as a photo and emailed to your child’s teacher.  Another option is to scan and email the form to your child’s teacher, or drop it off by the school in the front office.  Hard copies of the Virtual Learning Request Form are also available in the front office.
